Combined MitraClip® implantation and left atrial appendage occlusion using the Watchman™ device

Transcatheter mitral valve repair (TMVR) using the MitraClip® system is an emerging approach to treat selected cases of degenerative and functional mitral regurgitation (MR). Patients referred for MitraClip® implantation often have concomitant atrial fibrillation (AF), in addition to a frailty profile that represents an increased risk of thromboembolic and hemorrhagic events. Occlusion of the left atrial appendage (LAA) is currently considered an alternative for oral anticoagulation in patients with non-valvular AF.

We describe here a combined implantation using the MitraClip® system and the Watchman™ device for the treatment of primary MR and LAA occlusion. A 77-year-old female patient with history of breast cancer treated with surgery and radiation therapy, previous thromboembolism, permanent atrial fibrillation treated with oral anticoagulant (OAC) (edoxaban), presented with an important MR due to myxomatous degeneration and angiodysplasia of the colon, with several episodes of intestinal bleeding.
